Your Responsibilities

• Evaluate patients and establish individualized treatment plans

• Perform reassessments and discharge visits

• Deliver skilled physical therapy services in the home setting

• Educate patients and caregivers on mobility, safety, and recovery goals

• Coordinate care with physicians and interdisciplinary team members

• Maintain accurate and timely documentation

Qualifications

• Active Virginia Physical Therapist License Required

• Valid Driver's License and Reliable Transportation

• Excellent Communication and Documentation Skills

• Home Health Experience Preferred but Not Required
