Position Overview:

We are seeking an experienced Home Health Physical Therapist to join our dynamic team. The ideal candidate will be passionate about improving the quality of life for patients by providing expert, individualized physical therapy services. This role offers the opportunity to work independently while collaborating with an interdisciplinary team to deliver the highest standard of care.

 

Responsibilities:

 

  1. Conduct comprehensive assessments of patients’ physical and functional abilities.
  2. Develop personalized treatment plans based on physician recommendations and patient goals.
  3. Provide skilled physical therapy interventions to improve mobility, strength, balance, and overall function.
  4. Educate patients and caregivers on therapeutic exercises, safety precautions, and injury prevention.
  5. Monitor progress and adjust treatment plans to optimize outcomes.
  6. Document clinical findings, interventions, and patient responses accurately and promptly.
  7. Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.
  8. Maintain compliance with all regulatory standards, including HIPAA and Medicare guidelines.
  9. Participate in ongoing professional development to stay current with best practices.

 

 

Qualifications:

 

  1. Graduate of an accredited physical therapy program with an active Virginia license.
  2. Minimum of 3 years’ clinical experience, preferably in home health or skilled nursing facilities.
  3. Strong clinical reasoning, assessment, and treatment planning skills.
  4. Experience working with diverse patient populations, including adults and older adults.
  5. Ability to recognize medical emergencies and respond appropriately.
  6. Excellent communication and interpersonal skills for working with patients, families, and care teams.
  7. Proficiency in electronic medical record (EMR) systems; Kinnser, WellSky, or Homecare Homebase preferred.
  8. OASIS experience preferred.
  9. Ability to supervise Physical Therapist Assistants (PTAs).
  10. Self-motivated with strong time management and organizational skills.
  11. Reliable transportation and a valid driver’s license.
