Position Overview

We are seeking a licensed Physical Therapist for full-time and PRN home health visits. This role is ideal for clinicians who value autonomy, meaningful patient relationships, and manageable caseloads.

Key Responsibilities

• Perform in-home physical therapy evaluations

• Develop and implement individualized treatment plans

• Provide one-on-one therapy to improve mobility, strength, and independence

• Educate patients and caregivers on exercises and safety

• Monitor progress and adjust care plans as needed

• Complete timely and accurate EMR documentation

• Collaborate with interdisciplinary care teams

• Supervise PTAs as required

• Ensure compliance with state and federal regulations

Qualifications

• Active Virginia Physical Therapist license

• Graduate of an accredited PT program

• Minimum 2 years of experience (home health or SNF preferred)

• Strong clinical and communication skills

• EMR experience preferred (WellSky or HCHB)

• OASIS experience a plus

• Valid driver’s license & reliable transportation
