Overview: We are seeking an experienced and compassionate Licensed Practical/Vocational Nurse (LPN/LVN) to join our home health care team. Responsibilities:

  1. Conduct comprehensive nursing assessments of patients in their homes, including physical, psychosocial, and environmental factors, to develop individualized care plans.
  2. Perform skilled nursing procedures and treatments as prescribed by the physician or advanced practice provider, such as wound care, medication administration, injections, catheter care, and monitoring vital signs.
  3. Monitor patients' health status, symptoms, and response to treatment, and communicate any changes or concerns to the supervising registered nurse or healthcare provider.
  4. Educate patients and caregivers on disease management, medication adherence, self-care techniques, and preventive measures to promote optimal health and independence.
  5. Provide emotional support, counseling, and reassurance to patients and families coping with illness, disability, or end-of-life issues, demonstrating empathy, compassion, and cultural sensitivity.
  6. Coordinate care with other members of the healthcare team, including physicians, therapists, social workers, and home health aides, to ensure holistic and coordinated care delivery.
  7. Document nursing assessments, interventions, and patient responses accurately and in accordance with regulatory standards and organizational policies, utilizing electronic health record (EHR) systems.
  8. Collaborate with patients, families, and caregivers to develop realistic goals and discharge plans, facilitating transitions of care and continuity of services as patients' needs evolve.
  9. Implement infection control practices, safety measures, and emergency protocols to protect patients, caregivers, and oneself from harm, adhering to established guidelines and protocols.
  10. Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to review patient progress, identify barriers to care, and enhance care coordination and outcomes.

Qualifications:

  1. Graduate of an accredited practical/vocational nursing program with a current license to practice as an LPN or LVN in the state of employment.
  2. Minimum of 2-3 years of nursing experience, preferably in home health, community health, or a related healthcare setting, demonstrating proficiency in skilled nursing procedures and clinical assessments.
  3. Strong clinical assessment skills, critical thinking abilities, and proficiency in administering medications, treatments, and procedures safely and effectively.
  4. Excellent communication, interpersonal, and patient education skills, with the ability to establish rapport with patients and families from diverse backgrounds and cultures.
  5. Knowledge of nursing principles, practices, and standards of care relevant to home health nursing, including regulatory requirements and best practices in community-based care.
  6. Ability to work autonomously, manage time effectively, and prioritize tasks in a dynamic and fast-paced home health environment.
  7. Proficiency in using electronic health record (EHR) systems and other technology platforms for documentation, communication, and care coordination.
  8. Valid driver's license, reliable transportation, and willingness to travel to patients' homes within the designated service area.
  9. CPR certification and other relevant certifications may be required, depending on state regulations and organizational policies.
  10. Commitment to professional growth, continuing education, and adherence to ethical standards of nursing practice, with a passion for providing compassionate and patient-centered care.
