Responsibilities

• Conduct psychosocial assessments

• Provide counseling and crisis intervention

• Coordinate care with interdisciplinary teams

• Assist with discharge planning

• Connect patients to community resources

• Maintain accurate, compliant documentation

Qualifications

• MSW from CSWE-accredited program

• Active LMSW or LCSW (or eligibility)

• 2+ years healthcare or medical social work experience

• Strong case management and communication skills

• Knowledge of HIPAA and ethical standards
