Job Overview:
As a Patient Care Coordinator, you will provide administrative and clinical support to the healthcare team, ensuring smooth patient scheduling, effective communication, and quality patient care. The role involves a combination of phone-based customer service, patient scheduling, assisting with chart audits, supporting Nurse Clinical Managers, and helping maintain inventory and clinical operations. You will be an essential point of contact for patients and families, assisting them with their needs and ensuring they receive timely and accurate information.
Key Responsibilities:
  • Patient Scheduling & Coordination:
    • Answer inbound calls from patients, caregivers, and providers, addressing inquiries or concerns.
    • Manage patient appointments, ensuring they are scheduled and rescheduled promptly and efficiently.
    • Confirm appointments and provide reminders to patients regarding their visits.
  • Patient Complaint & Call Management:
    • Triage patient and family calls to assess needs, resolve issues, and escalate concerns as needed.
    • Handle patient complaints in a professional, empathetic manner, working toward timely resolutions.
  • Clinical & Chart Support:
    • Assist with chart audits to ensure compliance with internal policies and regulatory standards.
    • Help maintain patient records by updating information as necessary and assisting the clinical team with documentation needs.
  • Nurse Clinical Manager Support:
    • Provide administrative assistance to Nurse Clinical Managers by coordinating schedules, patient follow-ups, and other duties as required.
    • Facilitate communication between clinical teams and patients to ensure coordinated care.
  • Inventory Management:
    • Assist in managing inventory of medical supplies and equipment, ensuring stock is adequate and replenished in a timely manner.
    • Help track usage of medical supplies and maintain accurate records.
  • N95 Fit Testing:
    • Administer N95 respirator fit testing for staff to ensure compliance with safety protocols and regulations.
  • Additional Administrative Support:
    • Perform general administrative duties as needed to support patient care operations and maintain efficient workflow.
    • Maintain a high level of organization to handle multiple tasks and requests simultaneously.

Requirements:
  • Licensure:
    • Current Licensed Practical Nurse (LPN) in good standing with the Maryland Board of Nursing.
  • Experience:
    • Minimum of 3 years of clinical experience, preferably in home health or a related healthcare setting.
    • Strong customer service experience and ability to communicate effectively with patients and families.
    • Management experience is preferred but not required.
  • Skills & Competencies:
    • Excellent organizational skills and the ability to manage multiple tasks efficiently.
    • Strong verbal and written communication skills.
    • Ability to prioritize tasks and handle patient information with confidentiality.
    • Ability to work independently and as part of a collaborative healthcare team.
    • Proficient in Microsoft Office Suite or similar office software.
  • Physical Requirements:
    • Ability to perform tasks requiring sitting, standing, and occasional lifting (up to 25 lbs).
    • Comfort with wearing personal protective equipment (PPE) such as N95 masks during certain tasks.
