As a Home Health Occupational Therapist, you'll help patients regain the skills, confidence, and independence needed to safely navigate everyday life—all while providing care in the comfort of their own homes.

Your expertise empowers patients to overcome challenges, adapt to life changes, and achieve goals they once thought were out of reach.

What You'll Do

• Complete occupational therapy evaluations, reassessments, and discharge visits
• Develop individualized treatment plans based on patient goals and functional needs
• Provide skilled occupational therapy services in patients' homes
• Assess home environments and recommend adaptive equipment and safety modifications
• Educate patients and caregivers on therapeutic techniques, home safety, and activities of daily living
• Monitor patient progress and modify treatment plans as appropriate
• Collaborate with physicians and interdisciplinary team members
• Complete timely and accurate clinical documentation

Qualifications

• Active Virginia Occupational Therapist License Required
• Valid Driver's License and Reliable Transportation
• Strong Clinical Assessment and Documentation Skills
• Excellent Communication and Interpersonal Skills
• Home Health Experience Preferred but Not Required
• Experience with OASIS is a plus
