Experienced Home Health Registered Nurse

Responsibilities:

  • Perform comprehensive nursing assessments of patients in their homes, including physical, psychosocial, and environmental factors.

  • Develop individualized care plans in collaboration with patients, families, physicians, and other members of the healthcare team.

  • Provide skilled nursing care such as wound care, medication management, infusion therapy, catheter care, and palliative care.

  • Monitor patients’ health status, symptoms, and responses, adjusting care plans and communicating changes to the interdisciplinary team.

  • Educate patients and caregivers on disease management, medication adherence, and self-care techniques.

  • Coordinate care with therapists, social workers, home health aides, and community resources.

  • Document assessments, interventions, and patient responses accurately in the EHR system.

  • Provide emotional support, counseling, and advocacy to patients and families, with compassion and cultural sensitivity.

  • Implement infection control practices, safety measures, and emergency protocols.

  • Participate in team meetings, case conferences, and quality improvement initiatives.

Qualifications:

  • Graduation from an accredited nursing program with current RN licensure in the state of employment.

  • Minimum 2–3 years of nursing experience, preferably in home health or community health. Experience as an LPN will be considered.

  • Strong clinical assessment skills and proficiency in administering nursing procedures.

  • Excellent communication and interpersonal skills.

  • Knowledge of home health nursing principles, standards of care, and regulatory requirements.

  • Ability to work independently, prioritize tasks, and manage time effectively.

  • Proficiency with electronic health records (EHR) systems.

  • Experience with wound management and diagnosis preferred.

  • Valid driver’s license, reliable transportation, and willingness to travel within the service area.
