Experienced Home Health Registered Nurse (RN)
Full-time, Part-time, and PRN Opportunities Available

Responsibilities

  • Perform comprehensive home health nursing assessments

  • Develop individualized care plans with patients, families, and providers

  • Provide skilled nursing care (wound care, medication management, infusion, catheter, palliative care)

  • Monitor patient progress and adjust care plans as needed

  • Educate patients/caregivers on disease management and self-care

  • Collaborate with interdisciplinary teams for holistic care

  • Document all care accurately in the EHR system

  • Provide emotional support and advocacy for patients and families

  • Maintain infection control, safety, and emergency protocols

  • Participate in team meetings and quality improvement efforts

Qualifications

  • Current RN license in the state of employment

  • 2–3 years of nursing experience (home health preferred; LPN experience considered)

  • Strong clinical assessment, critical thinking, and communication skills

  • Proficient in skilled nursing procedures, patient education, and EHR use

  • Experience with wound management preferred

  • Valid driver’s license and reliable transportation

  • Commitment to compassionate, patient-centered care
