Responsibilities:
- Perform comprehensive nursing assessments of patients in their homes, including physical, psychosocial, and environmental factors, to identify needs, strengths, and goals for care.
- Perform comprehensive initial assessments and hand off to the LPN. Completing other Oasis visits as needed for Re certification or Discharge.
- Develop individualized care plans in collaboration with patients, families, physicians, and other members of the healthcare team, incorporating evidence-based practices and patient preferences.
- Provide skilled nursing care and interventions as prescribed by physicians or advanced practice providers, such as wound care, medication management, infusion therapy, catheter care, and palliative care.
- Monitor patients' health status, symptoms, and response to treatment, and adjust care plans accordingly, communicating any changes or concerns to the interdisciplinary team.
- Educate patients and caregivers on disease management, medication adherence, self-care techniques, and preventive measures to promote optimal health and independence.
- Coordinate care with other healthcare professionals, including therapists, social workers, home health aides, and community resources, to ensure holistic and coordinated care delivery.
- Document nursing assessments, interventions, and patient responses accurately and in accordance with regulatory standards and organizational policies, utilizing electronic health record (EHR) systems.
- Provide emotional support, counseling, and advocacy to patients and families coping with illness, disability, or end-of-life issues, demonstrating empathy, compassion, and cultural sensitivity.
- Implement infection control practices, safety measures, and emergency protocols to protect patients, caregivers, and oneself from harm, adhering to established guidelines and protocols.
- Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to review patient progress, identify barriers to care, and enhance care coordination and outcomes.
Qualifications:
- Graduation from an accredited nursing program with current licensure as a Registered Nurse (RN) in the state of employment.
- Minimum of 2-3 years of nursing experience, preferably in home health, community health, or a related healthcare setting, demonstrating proficiency in skilled nursing procedures and clinical assessments. Experience as a LPN will be considered.
- Strong clinical assessment skills, critical thinking abilities, and proficiency in administering medications, treatments, and procedures safely and effectively.
- Excellent communication, interpersonal, and patient education skills, with the ability to establish rapport with patients and families from diverse backgrounds and cultures.
- Knowledge of nursing principles, practices, and standards of care relevant to home health nursing, including regulatory requirements and best practices in community-based care.
- Ability to work autonomously, manage time effectively, and prioritize tasks in a dynamic and fast-paced home health environment.
- Proficiency in using electronic health record (EHR) systems and other technology platforms for documentation, communication, and care coordination.
- Experience with wound management and diagnosis preferred
- Valid driver's license, reliable transportation, and willingness to travel to patients' homes within the designated service area.
