What You’ll Do

• Perform comprehensive in-home patient assessments

• Develop and implement individualized care plans

• Provide skilled nursing care including wound care, medication management, and disease education

• Complete accurate and timely documentation (OASIS as applicable)

• Educate patients and caregivers on disease management and safety

• Coordinate care with physicians and interdisciplinary team members

• Monitor patient progress and communicate changes appropriately

What We’re Looking For

• Active Washington, DC Registered Nurse (RN) license

• Minimum 1–2 years nursing experience (home health preferred)

• OASIS experience preferred (if skilled role)

• EMR experience (WellSky/Kinnser or Homecare Homebase a plus)

• Strong assessment and documentation skills

• Reliable transportation
