Position: Experienced Home Health Licensed Practical/Vocational Nurse (LPN/LVN) – Fredericksburg, VA

Responsibilities:

• Conduct comprehensive nursing assessments of patients in their homes, evaluating physical, psychosocial, and environmental factors to develop individualized care plans.

• Perform skilled nursing procedures and treatments as prescribed by physicians or advanced practice providers (e.g., wound care, medication administration, injections, catheter care, vital signs monitoring).

• Monitor patients’ health status, symptoms, and response to treatment; promptly communicate changes or concerns to the supervising RN or provider.

• Educate patients and caregivers on disease management, medication adherence, self-care techniques, and preventive health practices.

• Provide compassionate emotional support to patients and families, especially those coping with illness, disability, or end-of-life needs.

• Coordinate care with physicians, therapists, social workers, and home health aides to ensure a holistic and seamless care experience.

• Accurately document assessments, interventions, and outcomes in accordance with regulations and organizational policies using EHR systems.

• Collaborate with patients, families, and caregivers to set achievable goals, develop discharge plans, and support smooth care transitions.

• Maintain infection control practices, safety measures, and emergency protocols for patient and staff protection.

• Participate in interdisciplinary team meetings, case conferences, and quality initiatives to promote excellence in patient care.

Qualifications:

• Graduate of an accredited practical/vocational nursing program with current state LPN/LVN license.

• Minimum 2–3 years of nursing experience (home health or community health preferred).

• Strong clinical assessment skills, critical thinking, and safe practice in skilled nursing procedures.

• Excellent communication and teaching skills with patients and families from diverse backgrounds.

• Knowledge of home health nursing principles, best practices, and regulatory requirements.

• Ability to work independently, prioritize effectively, and adapt in a fast-paced environment.

• Proficiency with electronic health records (EHR) and care coordination tools.

• Valid driver’s license, reliable transportation, and willingness to travel within Fredericksburg and surrounding areas.
