Responsibilities
-
Perform comprehensive nursing assessments of patients in their homes, including physical, psychosocial, and environmental factors.

Develop individualized care plans in collaboration with patients, families, physicians, and other members of the healthcare team.

Provide skilled nursing care and interventions as prescribed (e.g., wound care, medication management, infusion therapy, catheter care, palliative care).

Monitor patients’ health status, symptoms, and response to treatment, adjusting care plans as needed and communicating changes promptly.

Educate patients and caregivers on disease management, medication adherence, self-care, and preventive health practices.

Coordinate care with therapists, social workers, home health aides, and community resources to ensure holistic service delivery.

Document assessments, interventions, and outcomes accurately in the EHR in line with regulatory and organizational standards.

Provide emotional support and advocacy to patients and families coping with illness, disability, or end-of-life issues.

Maintain strict infection control and safety practices in accordance with established protocols.

Participate in interdisciplinary meetings, case conferences, and quality improvement initiatives.
Qualifications

Graduate of an accredited nursing program with current licensure as a Registered Nurse (RN) in the state of employment.

2–3 years of nursing experience preferred (home health or community health strongly valued). Experience as an LPN will be considered.

Strong clinical assessment, critical thinking, and technical nursing skills.

Excellent communication and patient education skills with cultural sensitivity.

Knowledge of home health regulations, standards of care, and best practices.

Ability to work independently and manage a dynamic caseload effectively.

Proficiency in electronic health record (EHR) systems.

Experience with wound management and diagnosis preferred.

Valid driver’s license, reliable transportation, and willingness to travel within the service area.
