Overview: We are seeking a dedicated and compassionate Physical Therapy Assistant (PTA) to join our home health care team. As a Home Health Physical Therapy Assistant, you will work under the supervision of a licensed Physical Therapist to provide rehabilitative services to patients in their homes. Your role will involve implementing individualized treatment plans, assisting patients with therapeutic exercises and activities, and promoting their functional independence and mobility. Responsibilities:

  1. Assist the supervising Physical Therapist in conducting initial and ongoing assessments of patients' physical abilities, functional status, and rehabilitation needs.
  2. Collaborate with the interdisciplinary team to develop comprehensive treatment plans based on patients' goals, medical history, and home environment.
  3. Implement therapeutic interventions and exercises prescribed by the Physical Therapist to improve patients' strength, range of motion, balance, coordination, and mobility.
  4. Provide hands-on assistance and guidance to patients during therapeutic exercises, gait training, transfers, and activities of daily living (ADLs) to maximize their functional outcomes.
  5. Educate patients and caregivers on proper techniques, safety precautions, and home exercise programs to support rehabilitation progress and prevent re-injury.
  6. Utilize therapeutic modalities, adaptive equipment, and assistive devices as directed by the Physical Therapist to enhance treatment effectiveness and promote independence.
  7. Document treatment sessions, progress notes, and patient responses accurately and in accordance with regulatory and organizational guidelines.
  8. Communicate effectively with the supervising Physical Therapist, other healthcare team members, and family members to coordinate care, address concerns, and promote continuity of care.
  9. Monitor patients' responses to treatment, report any changes or concerns to the Physical Therapist, and assist in modifying treatment plans as needed to achieve desired outcomes.
  10. Maintain professionalism, ethical standards, and patient confidentiality at all times, while adhering to state licensure requirements and organizational policies.

Qualifications:

  1. Graduation from an accredited Physical Therapist Assistant program with an Associate's degree in Physical Therapy Assisting.
  2. Current Maryland state licensure or eligibility for licensure as a Physical Therapy Assistant.
  3. Previous experience in a healthcare setting, preferably in home health, rehabilitation, or geriatrics for a minimum of 2 years.
  4. Strong interpersonal skills and the ability to establish rapport with patients, families, and caregivers.
  5. Demonstrated proficiency in assisting with therapeutic exercises, manual therapy techniques, and functional training activities.
  6. Knowledge of anatomy, physiology, kinesiology, and principles of physical therapy practice.
  7. Ability to follow treatment plans, exercise protocols, and safety guidelines under the supervision of a licensed Physical Therapist.
  8. Excellent communication, organization, and time-management skills, with attention to detail and accuracy in documentation.
  9. Valid driver's license and reliable transportation for home visits within the designated service area.
  10. CPR certification and other relevant certifications may be required, depending on state regulations and organizational policies.
