Home health allows you to see patients where they are most comfortable—their homes. You'll help individuals recover from illness, injury, surgery, and hospitalization while empowering them to live safely and independently.

Your care won't be measured by how many patients you can see in a day.

What You'll Do

• Evaluate patients and establish individualized treatment plans
• Provide skilled physical therapy services in the home setting
• Assess patient progress and adjust treatment plans as needed
• Educate patients and caregivers on mobility, safety, and recovery goals
• Collaborate with physicians and interdisciplinary team members
• Complete timely and accurate documentation

What We're Looking For

• Active Virginia Physical Therapist License
• Valid Driver's License and Reliable Transportation
• Strong Clinical, Communication, and Documentation Skills
• Home Health Experience Preferred
• Passion for Patient-Centered Care
