Description

Human Touch Home Health is seeking a HR/ Patient Care Coordinator for our Fredericksburg, VA office.

PCC Duties:
1. Accurately enters a variety of data into patient information systems.
2. Assists the functions of billing, payroll, human resource management, and patient care management through timely and accurate data entry and system reporting
3. Verifies accuracy of data entered through use of information systems reports.
4. Assists the Director of Nursing and Clinical Manager and management team members in the preparation of monthly management, statistical data.
5. Alerts appropriate management team members regarding late or missing documents required for data entry.
6. Alerts appropriate members of the information system support personnel to resolve system problems that negatively impact the speed and accuracy of data entry functions.
7. Performs other specific projects relating to data entry and computer operations as required.
8. Always maintains confidentiality of patient and organization information.
9. Consults with physicians, nurses, social workers, discharge planners and other disciplines to establish a coordinated home plan of care.
10. Attends meetings and participates on committees as required.
11. Performs other duties as assigned.
12. Manages all aspects of patient orders including put not limited to: getting orders signed in a timely manner, making sure the orders are in the patients electronic and physical chart, and notifies clinicians of new patient orders.
 

 

HR Duties:
1. Directs all daily human resources operations including providing direct oversight of the establishment and implementation of human resource policies.
2. Ensures compliance with all state, federal, and Joint Commission human resources regulatory requirements.
3. Directs the implementation of improved work methods and procedures to ensure that consistent employee recruitment, selection, retention, hiring practices, orientation and termination procedures are handled in accordance with applicable law and regulation.
4. Works with the management team to identify recruitment needs and develops a recruitment plan designed to fill demand and evaluates the cost-effectiveness of recruitment efforts.
5. Builds and monitors community perceptions of Human Touch Home Health as a high-quality employer.
6. Works with the management team in identifying employee education needs and develops orientation and continuing education programs for all employees.
7. Maintains a comprehensive library of resources to support employee education and development
8. Maintains confidentiality of all employee information and files.
9. Annually monitors personnel satisfaction with benefits and work environment and reports the results per organization practice.

Education and Experience Requirements:

  • At least two years experience in health care data entry, preferably in home health care information systems.
  • Able to use compatible systems and equipment.
  • Is a high school graduate or equivalent, Bachelor’s degree preferred
