Responsibilities:

• Perform in-home nursing assessments

• Develop and manage patient care plans

• Provide skilled nursing care including wound care, medication management, infusion therapy, catheter care, and palliative care

• Educate patients and caregivers

• Coordinate care with physicians and interdisciplinary team

• Document care accurately in EHR

• Follow safety and infection control standards

Qualifications:

• Active RN license

• 2–3 years nursing experience (home health preferred)

• Strong clinical and communication skills

• EHR experience

• Valid driver’s license and reliable transportation
