Human Touch Home Health is looking for a compassionate Occupational Therapist (OT) for PRN home health opportunities in Fredericksburg and surrounding communities.

Patient opportunities may be available in Fredericksburg, Stafford, Spotsylvania, King George, Locust Grove, Caroline County, and surrounding areas.

What You'll Do

• Complete occupational therapy evaluations in patients' homes

• Develop individualized plans of care

• Help patients improve independence with activities of daily living

• Provide therapeutic interventions to improve strength, coordination, mobility, and functional ability

• Assist patients with dressing, bathing, grooming, toileting, meal preparation, and other daily activities

• Assess home safety and recommend adaptive equipment or modifications

• Educate patients, families, and caregivers

• Monitor progress and adjust treatment plans as needed

• Complete reassessments and discharge planning

• Coordinate care with physicians, nurses, therapists, and other members of the care team

• Complete accurate and timely documentation using WellSky

What We're Looking For

• Active Virginia Occupational Therapist License Required

• Graduate of an accredited Occupational Therapy program

• Valid Driver's License and reliable transportation

• Strong clinical, communication, and organizational skills

• Ability to work independently in patients' homes

• Home Health Experience Preferred, but Not Required

• WellSky Experience is a Plus
