Human Touch Home Health is hiring a PRN Physical Therapist to serve patients throughout Fairfax, Loudoun, and Prince William Counties, Virginia.

This role is ideal for a Physical Therapist who wants flexibility, independence, and the ability to provide one-on-one care in the home setting.

What You’ll Do

• Perform physical therapy evaluations and reassessments
• Develop and implement individualized treatment plans
• Provide skilled therapy services in patients’ homes
• Coordinate care with physicians and the clinical team
• Complete documentation accurately and on time

Requirements

• Active Virginia Physical Therapist license
• Current CPR certification preferred
• Valid driver’s license and reliable transportation
• Strong communication and documentation skills
• Home health experience preferred, but not required
