What You'll Do

• Complete occupational therapy evaluations in patients' homes
• Develop individualized treatment plans
• Help patients improve independence with activities of daily living
• Provide therapeutic interventions to improve strength, coordination, mobility, and function
• Assess home safety and recommend modifications or adaptive equipment
• Educate patients and caregivers on safety and daily living techniques
• Monitor patient progress and adjust treatment plans as needed
• Coordinate care with physicians, nurses, therapists, and other team members
• Complete progress assessments and discharge planning
• Complete accurate and timely documentation using WellSky

Qualifications

• Active Washington, DC Occupational Therapist License Required
• Graduate of an accredited Occupational Therapy program
• Strong clinical assessment and communication skills
• Dependable transportation to patient visits
• Ability to work independently in patients' homes
• Home Health and OASIS Experience Preferred, but Not Required
• WellSky Experience is a Plus
