What You’ll Do:

• Conduct social work assessments in patients’ homes
• Identify social, emotional, and environmental needs
• Provide counseling and emotional support to patients and families
• Assist with community resources, discharge planning, and care coordination
• Support patients with housing, financial, and support services
• Collaborate with nurses, therapists, and physicians
• Complete accurate and timely EMR documentation

What We’re Looking For:

• Licensed Social Worker (LMSW or LCSW) in DC, Maryland, or Virginia (or eligible)
• Minimum of 1 year of clinical or medical social work experience required
• Home health or healthcare experience preferred
• Strong communication, empathy, and problem-solving skills
• Knowledge of community resources and care coordination
• Ability to work independently and manage a flexible schedule
