As a Home Health Social Worker, you'll help patients and families navigate some of life's most challenging moments.

You'll connect individuals with community resources, provide emotional support, assist with long-term planning, and help remove barriers that may impact a patient's health, safety, and quality of life.

What You'll Do

• Complete psychosocial assessments for home health patients

• Identify social, emotional, financial, and environmental needs

• Connect patients and families with appropriate community resources

• Assist with discharge planning and long-term care planning

• Provide counseling, support, and education to patients and caregivers

• Collaborate with physicians and interdisciplinary team members

• Maintain timely and accurate documentation

Qualifications

• Active Virginia Licensed Master Social Worker (LMSW) License Required

• Master's Degree in Social Work from an Accredited Program

• Strong Communication and Interpersonal Skills

• Excellent Organizational and Documentation Skills

• Valid Driver's License and Reliable Transportation

• Home Health or Healthcare Experience Preferred
