Position Overview

We are seeking an experienced Registered Nurse (RN) Case Manager to support clinical operations for our Silver Spring office.

This role is ideal for an RN who thrives in care coordination, clinical oversight, and interdisciplinary collaboration. You will ensure high-quality, patient-centered home health services while supporting field staff and maintaining regulatory compliance.


Key Responsibilities

  • Oversee and coordinate patient care plans in the home health setting

  • Conduct comprehensive assessments and reassessments

  • Develop and manage individualized care plans

  • Collaborate with physicians and interdisciplinary team members

  • Monitor patient outcomes and adjust care plans as needed

  • Provide clinical guidance and support to field staff

  • Ensure compliance with Medicare, state, and accreditation standards

  • Maintain timely and accurate documentation


Qualifications

  • Active Maryland RN License (Required)

  • Minimum 3 years of clinical experience (Home Health strongly preferred)

  • Case Management or Care Coordination experience preferred

  • Knowledge of Medicare regulations and home health standards

  • Strong communication and leadership skills

  • EMR experience preferred
