As a Home Health Physical Therapist, you'll provide individualized care in the comfort of your patients' homes, helping them regain strength, restore mobility, improve balance, and return to the activities that matter most.

You'll have the autonomy to develop personalized treatment plans while building meaningful relationships with patients and their families.

What You'll Do

• Complete evaluations, reassessments, and discharge visits
• Develop individualized plans of care based on patient goals and clinical needs
• Provide skilled physical therapy services in patients' homes
• Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, and home safety
• Monitor patient progress and modify treatment plans as needed
• Collaborate with physicians and the interdisciplinary care team
• Complete timely and accurate clinical documentation using WellSky

Qualifications

• Active Washington, DC Physical Therapist License Required
• Valid Driver's License and Reliable Transportation
• Strong Clinical Assessment and Documentation Skills
• Excellent Communication and Interpersonal Skills
• Home Health Experience Preferred but Not Required
• OASIS Experience is a Plus
• Passion for Delivering Exceptional Patient-Centered Care
