Human Touch Home Health is seeking a Physical Therapist (PT) for PRN home health opportunities throughout Washington, DC.

What You'll Do
- Complete physical therapy evaluations in patients' homes
- Develop individualized plans of care
- Provide therapeutic exercise, gait, balance, transfer, and mobility training
- Assess home safety and fall risks
- Educate patients and caregivers
- Monitor progress and adjust treatment plans
- Complete progress assessments and discharge planning
- Coordinate care with physicians, nurses, PTAs, and other team members
- Complete accurate and timely documentation using WellSky
Qualifications
- Active Washington, DC Physical Therapist License Required
- Graduate of an accredited Physical Therapy program
- Strong clinical assessment and communication skills
- Dependable transportation to patient visits
- Ability to work independently in patients' homes
- Home Health and OASIS experience preferred, but not required
- WellSky experience is a plus
