Position Overview:
We are seeking an experienced and compassionate Speech Therapist (PRN) to join our dynamic home health team in Fredericksburg and surrounding areas. In this role, you’ll have the opportunity to make a meaningful impact by helping patients improve communication, cognitive, and swallowing functions—all in the comfort of their homes.

Key Responsibilities:

• Conduct thorough speech-language and cognitive-communication assessments.

• Develop and implement customized treatment plans in line with physician orders and patient goals.

• Provide skilled interventions focused on improving speech, voice, fluency, language, and swallowing abilities.

• Educate patients and caregivers on therapy techniques, safety, and wellness strategies.

• Track patient progress, revise plans as needed, and document all findings promptly and accurately.

• Collaborate closely with interdisciplinary team members, including nurses, physicians, and therapists.

• Ensure compliance with HIPAA, Medicare, and other regulatory standards.

• Stay current through ongoing professional development.

Qualifications:

• Graduate of an accredited Speech-Language Pathology program.

• Current state license to practice Speech Therapy in Virginia.

• Minimum of 2 years of clinical experience preferred, especially in home health or skilled nursing settings.

• Excellent clinical reasoning, assessment, and treatment planning skills.

• Strong communication and interpersonal abilities.

• Experience with EMR systems (Kinnser/WellSky or Homecare Homebase preferred).

• Familiarity with OASIS documentation is a plus.

• Must have reliable transportation and a valid driver’s license.
