You do NOT have to cover the entire territory.

What You'll Do

  • Complete occupational therapy evaluations in patients' homes

  • Develop individualized plans of care

  • Help patients regain independence with activities of daily living

  • Address bathing, dressing, grooming, toileting, meal preparation, and other daily activities

  • Provide therapeutic interventions to improve functional ability

  • Assess home safety and recommend adaptive equipment or modifications

  • Educate patients, families, and caregivers

  • Monitor progress and modify treatment plans as needed

  • Complete reassessments and discharge planning

  • Collaborate with the interdisciplinary care team

  • Complete accurate and timely documentation using WellSky

What We're Looking For

  • Active Maryland Occupational Therapist License Required

  • Graduate of an accredited Occupational Therapy program

  • Reliable transportation

  • Strong clinical, communication, and organizational skills

  • Ability to work independently in patients' homes

  • Home Health Experience Preferred, but Not Required

  • WellSky Experience is a Plus
