As a Home Health Physical Therapist, you'll help patients recover from illness, injury, surgery, and hospitalization while improving strength, mobility, balance, and confidence in their daily lives.

Your expertise helps patients remain safe in their homes, avoid unnecessary hospitalizations, and return to the activities they enjoy most.

What You'll Do

• Complete evaluations, reassessments, and discharge visits
• Develop individualized treatment plans based on patient goals and needs
• Provide skilled physical therapy services in patients' homes
• Educate patients and caregivers on exercise programs, mobility training, fall prevention, and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Collaborate with physicians and interdisciplinary team members
• Maintain timely and accurate clinical documentation

Qualifications

• Active Pennsylvania Physical Therapist License Required
• Valid Driver's License and Reliable Transportation
• Strong Clinical, Communication, and Documentation Skills
• Home Health Experience Preferred but Not Required
• Commitment to Delivering Exceptional Patient-Centered Care
