We are seeking experienced and compassionate Supervisory Registered Nurses (RN) to join our skilled home health team. Responsibilities

• Perform comprehensive assessments of patients’ medical, psychosocial, and environmental needs.

• Develop individualized care plans in collaboration with patients, families, and the interdisciplinary team.

• Deliver skilled nursing services, including wound care, infusion therapy, medication management, catheter care, and disease-specific education.

• Supervise and support LPNs, CNAs, and other direct care staff to ensure compliance with physician orders and quality standards.

• Monitor patient progress, document changes, and communicate with physicians and clinical leadership.

• Conduct Medicaid client evaluations, re-certifications, and ensure regulatory compliance.

• Participate in discharge planning to ensure continuity of care and patient safety.

Qualifications

• Graduate of an accredited Bachelor of Science in Nursing (BSN), Associate Degree in Nursing (ADN), or Nursing Diploma program.

• Current DC state licensure as a Registered Nurse (RN).

• Minimum of one year of experience as an RN, preferably with Medical/Surgical background in an acute care setting.

• Prior home health experience strongly preferred.

• Strong leadership, organizational, and communication skills.

• Computer literate and able to use electronic health records efficiently.
