We are seeking experienced and compassionate Supervisory Registered Nurses (RN) to join our skilled home health team. Responsibilities
-
Perform comprehensive assessments of patients’ medical, psychosocial, and environmental needs.

Develop individualized care plans in collaboration with patients, families, and the interdisciplinary team.

Deliver skilled nursing services, including wound care, infusion therapy, medication management, catheter care, and disease-specific education.

Supervise and support LPNs, CNAs, and other direct care staff to ensure compliance with physician orders and quality standards.

Monitor patient progress, document changes, and communicate with physicians and clinical leadership.

Conduct Medicaid client evaluations, re-certifications, and ensure regulatory compliance.

Participate in discharge planning to ensure continuity of care and patient safety.
Qualifications

Graduate of an accredited Bachelor of Science in Nursing (BSN), Associate Degree in Nursing (ADN), or Nursing Diploma program.

Current DC state licensure as a Registered Nurse (RN).

Minimum of one year of experience as an RN, preferably with Medical/Surgical background in an acute care setting.

Prior home health experience strongly preferred.

Strong leadership, organizational, and communication skills.

Computer literate and able to use electronic health records efficiently.
