As a Full-Time Home Health Physical Therapist (PT), you’ll provide one-on-one patient care — no clinic overload, no double-booking, no burnout cycle.

What You’ll Do:

• Provide home health physical therapy services to adult and geriatric patients
• Perform evaluations and develop individualized care plans
• Deliver rehabilitation services focused on strength, balance, and mobility
• Educate patients and caregivers on treatment plans
• Collaborate with physicians and interdisciplinary care teams
• Complete accurate and timely EMR documentation

What We’re Looking For:

• Licensed Physical Therapist (PT) in Virginia (or eligible)
• Doctor of Physical Therapy (DPT) required
• Minimum of 1–2 years of clinical experience required
• Home health experience preferred, not required
• Strong communication, organization, and time management skills
• Ability to manage a consistent caseload independently
