Human Touch Home Health is looking for a compassionate Physical Therapist (PT) for PRN home health opportunities in the rural communities surrounding Fredericksburg, VA.

We're especially looking for therapists interested in patients located within approximately 50 minutes of Fredericksburg, including areas such as King George, Colonial Beach, Bowling Green, Caroline County, Spotsylvania, Locust Grove, Orange, and surrounding rural communities.

What You'll Do

• Complete physical therapy evaluations in patients' homes

• Develop individualized plans of care

• Provide therapeutic exercise, gait, balance, transfer, and mobility training

• Help patients improve strength, endurance, and functional independence

• Assess home safety and identify fall risks

• Educate patients and caregivers

• Monitor progress and adjust treatment plans as needed

• Complete reassessments and discharge planning

• Coordinate care with the interdisciplinary team

• Complete accurate and timely documentation using WellSky

What We're Looking For

• Active Virginia Physical Therapist License Required

• Graduate of an accredited Physical Therapy program

• Valid Driver's License and reliable transportation

• Comfortable traveling to rural patient locations

• Strong clinical and communication skills

• Ability to work independently in patients' homes

• Home Health Experience Preferred, but Not Required

• WellSky Experience is a Plus
