Human Touch Home Health is looking for a compassionate Medical Social Worker (MSW) for PRN home health opportunities in Fredericksburg and surrounding communities.

Patient opportunities may be available in Fredericksburg, Stafford, Falmouth, Spotsylvania, Massaponax, Spotsylvania Courthouse, Garrisonville, and surrounding areas.

Sometimes what keeps a patient from succeeding at home has nothing to do with the clinical treatment plan.

It may be transportation, caregiver burnout, financial concerns, housing challenges, access to food, long-term care needs, or simply not knowing what resources are available.

What You'll Do

• Complete psychosocial assessments in patients' homes

• Identify social, emotional, financial, and environmental barriers affecting care

• Connect patients and families with community resources and support services

• Assist with long-term care and community-based resource planning

• Provide short-term counseling and emotional support

• Support patients and caregivers during difficult transitions

• Assist with care and discharge planning

• Help identify available financial and transportation resources

• Advocate for patient and family needs

• Collaborate with nurses, therapists, physicians, and other members of the care team

• Maintain accurate and timely documentation

What We're Looking For

• Master's Degree in Social Work Required

• Appropriate Virginia Social Work Licensure Required

• Strong assessment, communication, and problem-solving skills

• Knowledge of community resources and support programs

• Valid Driver's License and reliable transportation

• Ability to work independently in patients' homes

• Compassionate and comfortable supporting patients and families through challenging situations

• Home Health or Healthcare Social Work Experience Preferred
