Human Touch Home Health is hiring a licensed Physical Therapist to provide home health services in Woodbridge, Dale City, Lake Ridge, Dumfries, Montclair, Occoquan, Lorton, Springfield, Alexandria, and surrounding communities north of Triangle along the I-95 corridor.

What You’ll Do

• Complete evaluations, treatment plans, reassessments, and discharge planning
• Provide skilled therapy services in patients’ homes
• Coordinate care with physicians and the interdisciplinary team
• Manage your own schedule and daily route

Requirements

• Active Virginia Physical Therapist license
• Valid driver’s license and reliable transportation
• Strong documentation and communication skills
• Home health experience preferred
