As a Home Health Physical Therapist, you'll work directly with patients in their homes, helping them recover from surgery, illness, injury, hospitalization, and mobility challenges while restoring strength, balance, confidence, and independence.

What You'll Do

• Complete evaluations, reassessments, and discharge visits
• Develop individualized treatment plans based on patient goals and needs
• Provide skilled physical therapy services in patients' homes
• Educate patients and caregivers on mobility, exercise programs, fall prevention, and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Collaborate with physicians and interdisciplinary team members
• Maintain timely and accurate clinical documentation

Qualifications

• Active Physical Therapist License Required in the applicable service area
• Valid Driver's License and Reliable Transportation
• Strong Clinical, Communication, and Documentation Skills
• Home Health Experience Preferred but Not Required
• Commitment to Delivering Exceptional Patient-Centered Care
