Human Touch Home Health is seeking a licensed Occupational Therapist to provide home health services on a per diem (PRN) basis throughout Fairfax County, Loudoun County, and Prince William County, Virginia.

What You’ll Do

• Complete occupational therapy evaluations, reassessments, and discharge planning
• Develop treatment plans to improve activities of daily living and functional independence
• Provide skilled therapy services in patients’ homes
• Educate patients and caregivers on adaptive techniques and safety
• Coordinate care with physicians and the interdisciplinary team

Requirements

• Active Virginia Occupational Therapist license
• Valid driver’s license and reliable transportation
• Strong documentation and communication skills
• Home health experience preferred
