Position Overview:
We are currently hiring full-time, part-time, and PRN Licensed Practical Nurses (LPNs) to join our home health care team. As a key member of our care team, you will deliver skilled nursing services directly in patients’ homes, helping them maintain independence and quality of life.
Responsibilities:

Conduct comprehensive nursing assessments in patients’ homes to support individualized care plans.

Administer skilled nursing procedures and treatments such as wound care, medication administration, injections, catheter care, and vital sign monitoring.

Monitor patient progress and promptly communicate concerns to the supervising RN or provider.

Educate patients and caregivers on disease management, medications, and self-care to promote independence.

Provide compassionate emotional support to patients and families, especially during challenging health situations.

Collaborate with physicians, therapists, social workers, and home health aides for coordinated care.

Maintain accurate documentation in electronic health records (EHR).

Support safe care environments by adhering to infection control and emergency protocols.

Participate in team meetings, case conferences, and quality improvement initiatives.
Qualifications:
-
Graduate of an accredited practical/vocational nursing program.
-
Current LPN/LVN license in the state of employment.

2–3 years of nursing experience (home health or community health preferred).

Strong clinical assessment and critical thinking skills.

Excellent communication and patient education skills.

Proficiency with electronic health records (EHR) systems.

Valid driver’s license, reliable transportation, and willingness to travel within Prince George’s and Montgomery counties.
