Responsibilities:
- Conduct comprehensive psychosocial assessments of patients and their families to identify strengths, challenges, and support systems impacting their health and well-being.
- Provide crisis intervention, emotional support, and counseling to patients and families coping with medical diagnoses, treatment decisions, end-of-life care, and other life transitions.
- Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans that address patients' psychosocial needs, treatment goals, and discharge plans.
- Advocate for patients' rights, preferences, and access to appropriate healthcare services, including referrals to specialty care, financial assistance programs, housing resources, and community support agencies.
- Assist patients and families in navigating the healthcare system, understanding medical procedures, and making informed decisions about their care options.
- Facilitate communication and collaboration among healthcare providers, patients, families, and community resources to promote continuity of care and holistic well-being.
- Document assessments, interventions, care plans, and case management activities accurately and in accordance with regulatory standards and organizational policies.
- Participate in multidisciplinary team meetings, case conferences, and discharge planning discussions to contribute insights, advocate for patients' needs, and promote coordinated care.
- Stay informed about social, cultural, economic, and legislative issues affecting healthcare delivery, patient populations, and community resources, and integrate this knowledge into practice.
- Engage in ongoing professional development, training, and supervision to enhance clinical skills, cultural competence, and ethical practice in medical social work.
Qualifications:
- Master's degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE).
- Current state licensure or eligibility for licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW).
- Minimum of 2-3 years of clinical experience in medical social work, healthcare settings, or related fields, with demonstrated competence in psychosocial assessment, counseling, and case management.
- Strong knowledge of social work theories, principles, ethics, and best practices in healthcare settings, including trauma-informed care, patient advocacy, and cultural humility.
- Excellent communication, interpersonal, and collaboration skills, with the ability to establish rapport with diverse patient populations, families, and interdisciplinary team members.
- Proficiency in crisis intervention, counseling techniques, and solution-focused approaches to address patients' emotional, behavioral, and adjustment difficulties.
- Familiarity with community resources, social service agencies, government benefits, and healthcare financing options relevant to patients' needs and circumstances.
- Ability to work autonomously, prioritize tasks, and make sound decisions under pressure in a fast-paced healthcare environment.
- Compassion, empathy, and resilience in working with individuals experiencing medical crises, chronic illnesses, disabilities, or end-of-life issues.
- Adherence to professional standards of confidentiality, ethical conduct, and documentation integrity, in compliance with HIPAA regulations and social work licensure requirements.
