As a Home Health Licensed Master Social Worker, you'll play a vital role in helping patients and families navigate the emotional, financial, and social challenges that often accompany illness and recovery.

Your guidance helps patients remain safe at home, access valuable community resources, and face life's challenges with confidence and dignity.

What You'll Do

• Complete comprehensive psychosocial assessments
• Identify emotional, social, environmental, and financial barriers affecting patient care
• Connect patients and families with community resources and support services
• Assist with discharge planning and long-term care planning
• Provide counseling, education, crisis intervention, and emotional support
• Advocate for patients and coordinate services with community agencies
• Collaborate with physicians and the interdisciplinary care team
• Complete timely and accurate clinical documentation

Qualifications

• Active Virginia Licensed Master Social Worker (LMSW) License Required
• Master's Degree in Social Work from an Accredited Program
• Valid Driver's License and Reliable Transportation
• Strong Assessment, Communication, and Documentation Skills
• Home Health or Healthcare Experience Preferred but Not Required
• Compassionate, organized, and committed to delivering patient-centered care
