Position Overview:
We are seeking an experienced and dedicated Certified Nursing Assistant to join our home health care team. As an integral member of our interdisciplinary team, you will provide skilled nursing care to patients in their homes, promoting their independence, comfort, and overall well-being. Your role will involve following individualized care plans and coordinating care with other healthcare professionals to meet patients' needs. Responsibilities and Duties:
-
Provide service to individuals in their own homes and communities who need assistance caring for themselves due to old age, sickness, disability, or other conditions.

Assist with activities such as bathing, exercising, meal planning and preparation, dressing, positioning, transferring/ambulating, feeding, companionship, housekeeping/laundry, personal hygiene/grooming, transportation, shopping/errands/outings, toileting, incontinence care, and ensuring client safety.

Deliver care in a respectful and caring manner, adhering to relevant Agency policies and industry standards.

Assist with activities of daily living by serving meals, feeding patients as necessary, ambulating, turning, and positioning patients; and providing fresh water and nourishment.

Administer adjunct care such as enemas, douches, non-sterile dressings, surgical preps, ice packs, heat treatments, sitz and therapeutic baths; and apply restraints as needed.

Monitor patient stability by checking vital signs and weight; testing urine and recording intake and output information.

Ensure patient comfort by utilizing resources, transporting patients, answering call lights and requests, and reporting observations to the nursing supervisor.

Document actions and complete forms, reports, logs, and records as required.

Follow policies and procedures to maintain work operations.

Protect the organization's value by maintaining patient confidentiality.
Qualifications and Skills:

Must be licensed by the Department of Health, VA.

Must have a current negative TB test or be able to obtain one.

Must pass a background check.

Must have reliable transportation.
Experience:

At least 1 year of experience as a CNA/HHA (Preferred).
