Human Touch Home Health is seeking a Full-Time Physical Therapist (PT) to join our growing Fredericksburg-area therapy team.

Patient opportunities may include Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, Orange, Locust Grove, Bowling Green, Colonial Beach, and rural surrounding communities.

You do not have to cover the entire territory. We work to create manageable patient assignments and routes based on location and service needs.

What You'll Do

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Help patients improve strength, endurance, and functional independence
• Assess home safety and identify fall risks
• Educate patients and caregivers on mobility, safety, and home exercise programs
• Monitor patient progress and adjust treatment plans as needed
• Complete reassessments and discharge planning
• Coordinate care with physicians, nurses, therapists, and other team members
• Complete accurate and timely documentation using WellSky

What We're Looking For

• Active Virginia Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical assessment and communication skills
• Ability to independently manage a home health caseload
• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus
