What You'll Do

As a Skilled Home Health RN, you will:

• Perform comprehensive skilled nursing assessments

• Complete Start of Care, Resumption of Care, Recertification, and Discharge visits

• Complete OASIS assessments as required

• Develop and update individualized plans of care

• Provide wound care and monitor wound progression

• Complete medication reconciliation and provide medication education

• Monitor patients for changes in condition

• Provide disease-management education

• Educate patients, families, and caregivers

• Communicate with physicians regarding changes in patient status

• Obtain and implement physician orders as appropriate

• Coordinate care with therapists, social workers, and other members of the interdisciplinary team

• Identify concerns early and help reduce avoidable hospitalizations

• Complete accurate and timely clinical documentation

• Ensure care is provided according to physician orders, agency policies, and applicable regulations

What We're Looking For

• Active Washington, DC Registered Nurse License Required

• Graduate of an accredited nursing program

• Strong nursing assessment and clinical decision-making skills

• Experience providing skilled nursing care

• Home Health and OASIS Experience Preferred

• Strong communication and documentation skills

• Excellent organizational and time-management abilities

• Ability to work independently in patients' homes

• Reliable transportation and ability to travel between assigned patient locations
