Human Touch Home Health is seeking an experienced Occupational Therapist (OT) for a PRN (as-needed) home health position serving Allentown, Bethlehem, Easton, Carbon County, and the greater Lehigh Valley region.

What You’ll Do

• Perform comprehensive in-home OT evaluations and reassessments

• Develop and implement individualized treatment plans

• Provide skilled interventions to improve functional independence

• Educate patients and caregivers on adaptive techniques and home safety

• Complete accurate and timely documentation (OASIS as applicable)

• Collaborate with physicians and interdisciplinary care team members

What We’re Looking For

• Active Pennsylvania Occupational Therapist license

• Minimum 1–2 years OT experience (home health preferred)

• OASIS experience preferred

• EMR experience (WellSky/Kinnser or Homecare Homebase a plus)

• Strong documentation and communication skills

• Valid driver’s license & reliable transportation
