Practice Physical Therapy With Purpose

As a Home Health Physical Therapist, you'll have the opportunity to work directly with patients where progress matters most—their homes.

What You'll Do

• Complete evaluations, reassessments, and discharge visits
• Develop individualized treatment plans based on patient goals and needs
• Provide skilled physical therapy services in patients' homes
• Educate patients and caregivers on mobility, exercise programs, fall prevention, and safety strategies
• Collaborate with physicians and interdisciplinary team members
• Maintain timely and accurate clinical documentation

Qualifications

• Active Maryland Physical Therapist License Required
• Valid Driver's License and Reliable Transportation
• Strong Clinical, Communication, and Documentation Skills
• Home Health Experience Preferred but Not Required
• Commitment to Delivering Exceptional Patient-Centered Care
