Human Touch Home Health is seeking a licensed Physical Therapist (PT) for a Full-Time home health position serving patients in Blacksburg, Virginia and surrounding communities.

Position Overview

The Physical Therapist will provide skilled therapy services in the home setting, supporting patients in improving mobility, strength, balance, and overall function. This role requires strong clinical judgment and coordination with the care team.

Responsibilities

• Perform in-home PT evaluations and reassessments

• Develop and implement individualized treatment plans per physician orders

• Provide skilled therapeutic interventions

• Complete documentation accurately and in a timely manner

• Educate patients and caregivers on home exercise programs and safety

• Collaborate with physicians and interdisciplinary team members

• Supervise Physical Therapist Assistants as applicable

Qualifications

• Active Virginia Physical Therapist license

• Minimum 1 year of experience (home health preferred)

• OASIS experience preferred

• Strong clinical and documentation skills

• EMR experience preferred

• Valid driver’s license and reliable transportation
