What You’ll Do

• Assess patients’ social, emotional, and financial needs
• Connect patients and families with community resources and support services
• Assist with long-term planning, counseling, and crisis intervention
• Coordinate care with physicians, nurses, therapists, and the interdisciplinary team
• Complete documentation accurately and on time

Requirements

• Active Virginia Licensed Master Social Worker (LMSW) license
• Valid driver’s license and reliable transportation
