As a Home Health Occupational Therapist, you'll work directly with patients in their homes, helping them improve their ability to perform daily activities safely and independently.

From recovering after surgery to adapting to chronic conditions, your expertise will help patients regain confidence and improve their quality of life.

What You'll Do

• Complete occupational therapy evaluations, reassessments, and discharge visits

• Develop individualized treatment plans

• Provide skilled occupational therapy services in patients' homes

• Assess home safety and recommend adaptive equipment as needed

• Educate patients and caregivers on techniques to improve independence

• Collaborate with physicians and interdisciplinary team members

• Maintain timely and accurate documentation

Qualifications

• Active Virginia Occupational Therapist License Required

• Valid Driver's License and Reliable Transportation

• Strong Communication and Documentation Skills

• Home Health Experience Preferred but Not Required

• Commitment to Delivering Patient-Centered Care
