Job Responsibilities

• Perform in-home nursing assessments and skilled visits

• Complete OASIS start of care, recertification, and discharge assessments

• Develop and update individualized care plans

• Provide skilled nursing care including wound care, medication management, IV therapy, and catheter care

• Educate patients and caregivers

• Coordinate care with interdisciplinary team members

• Maintain accurate and timely documentation in the EHR

• Follow infection control and safety standards

Qualifications

• Active Virginia RN license (LPN experience may be considered)

• 2–3 years of nursing experience (home health preferred)

• Experience with wound care and nursing diagnoses preferred

• Strong communication and clinical skills

• Valid driver’s license and reliable transportation
