Position Overview

We are seeking dedicated Non-Skilled Registered Nurses (RN) to provide Medicaid home health services to clients in Washington, D.C. This role involves visiting multiple patients per day in their homes and delivering individualized nursing care based on assessed needs and physician orders.

Responsibilities

• Conduct in-home nursing assessments

• Develop and implement individualized care plans

• Provide non-skilled nursing services (e.g., medication management, skin care, nutrition monitoring)

• Monitor patient conditions and report changes to physicians and supervisors

• Coordinate care with interdisciplinary team members

• Participate in discharge planning as needed

• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing program (ADN, BSN, or Diploma)

• Active RN license in the District of Columbia

• Minimum 2 year RN experience (home health preferred)

• Strong assessment, communication, and time-management skills

• Computer literacy required

• Reliable transportation for home visits
