Position Overview

We are seeking an experienced Home Health Physical Therapist Assistant (PTA) to support patients in their homes. This role is ideal for a motivated PTA who values independence, collaboration, and making a real impact on patient mobility and quality of life.

Key Responsibilities

• Assist the supervising Physical Therapist with patient evaluations and ongoing assessments

• Carry out prescribed treatment plans, therapeutic exercises, and mobility training

• Provide hands-on assistance with gait training, transfers, balance, and ADLs

• Educate patients and caregivers on home exercise programs and safety techniques

• Monitor patient progress and report changes to the Physical Therapist

• Document visits accurately and timely per regulatory and agency standards

• Collaborate with the interdisciplinary care team to ensure continuity of care

Qualifications

• Associate’s degree from an accredited Physical Therapist Assistant program

• Current Virginia PTA license or eligibility

• Minimum 2 years of experience, preferably in home health, rehab, or geriatrics

• Strong clinical, communication, and documentation skills

• Knowledge of anatomy, kinesiology, and physical therapy principles

• Valid driver’s license and reliable transportation
