As a Home Health Physical Therapist Assistant, you'll help patients regain strength, improve mobility, increase confidence, and achieve goals that make a real difference in their daily lives.

What You'll Do

• Provide skilled physical therapy treatments under the direction of a Physical Therapist
• Implement individualized treatment plans
• Monitor patient progress and communicate findings to the supervising therapist
• Educate patients and caregivers on mobility, exercises, and safety strategies
• Assist patients in achieving rehabilitation goals
• Maintain timely and accurate documentation
• Collaborate with physicians and interdisciplinary team members

Qualifications

• Active Virginia Physical Therapist Assistant License Required
• Valid Driver's License and Reliable Transportation
• Strong Communication and Documentation Skills
• Home Health Experience Preferred but Not Required
• Commitment to Delivering Exceptional Patient Care
